What is the Nursing Home Transition Services Program?

The program provides coordinated resources and services for adults to live independently in the community. NHT (Nursing Home Transition) services promote individual choice in long term living options

The agency serves as the facilitator of NHT services for individuals with disabilities under the age of 60 and has a collaborative relationship with the Chester County Office of Aging for long term living services.

Components of Nursing Home Transition Services


  • Counseling: Experienced staff familiar with level of care assessments and knowledge of options and resources in the commmunity. Nursing facility residents (consumers) can receive information about the wide variety of services available in the community to meet their long term living needs.

  • Education Staff provides individuals and their families with the necessary information about options so that individuals are making informed choices and avoiding unnecessary long term nursing facility placement.

  • Access of Community Options: Identify community based resources that may be needed to maintain independence while ensuring safety and quality living. Assistance in coordination of these services to support the nursing facility resident to transition ino the community.

  • Partnerships: Networking with community resources to facilitate consumer choice.

  • Relocation: Staff work with consumers, nursing facility staff and all community partners to assure transition is complete in a manner that assures consumer's safety while addressing all of their needs.

Community Transitions Services can include:


Enrollment in CSPPPD Waivers (Community Service Program for Persons with Physical Disabilities) to provide daily living services, community integration, Personal Emergency Response Systems, home modifications, transportation and Supports Coordination.

  • Moving expenses
  • Security deposit for rent and utilities
  • Essential furninshings

What are the costs?

There is no cost for these services. The Agency is a Medical Assistance Provider. Agency Programs are funded in part from government contracts, United Way member support, and private donations.

Who is eligible?


Any individual residing in Chester County in a nursing home who would like to return to the community.
